Background and Manufacturer Overview

Neodent Grand Morse (GM)

Developed in Brazil by Neodent (Curitiba, 1993) and now a flagship Straumann Group brand. The Grand Morse connection was introduced in 2010 as Neodent's proprietary evolution of the Morse taper concept. Today the GM system is one of the best-selling implant lines in Latin America and is rapidly expanding in the US market. Manufactured to ISO 13485 in Brazil, with CE and FDA 510(k) clearance.

Straumann BLT (Bone Level Tapered)

Launched in 2012 by Institut Straumann AG (Basel, Switzerland) as the tapered variant of the bone-level platform, complementing the parallel-walled BL system. Combines Straumann's SLActive surface technology and CrossFit internal connection in a tapered geometry. Carries one of the largest bodies of prospective Level I evidence in implantology. Premium-tier Swiss manufacturing.

Connection Design

Neodent Grand Morse — 16° Conical Connection

The Grand Morse connection is a self-locking 16° Morse taper (measured from the long axis). When the abutment is fully seated, the metallic cold-weld friction fit creates a biological seal that effectively eliminates the micro-gap at the implant-abutment interface. Independent studies using scanning electron microscopy have confirmed near-zero micro-gap formation at ISQ-comparable clamping forces. The connection is internal, anti-rotational (indexed with a spline inside the cone), and platform-switched by design — the abutment shoulder sits medially to the implant platform rim.

Straumann CrossFit — Internal Conical Connection

The CrossFit connection features a 15° internal taper combined with an anti-rotation mechanism (12-point internal geometry). It is also platform-switched. The CrossFit was engineered for the bone-level platform and has accumulated over 10 years of prospective clinical data. While the cone angle is slightly narrower than the Grand Morse (15° vs 16°), both are within the self-locking range and perform equivalently in micro-gap prevention in peer-reviewed head-to-head testing.

Material Grade IV commercially pure titanium (cpTi) Roxolid (Ti-Zr alloy) standard; cpTi also available

The Straumann BLT's use of Roxolid — a titanium-zirconium alloy with approximately 15% greater tensile strength than Grade IV titanium — is a meaningful clinical differentiator. In narrow-diameter scenarios (3.3 mm), Roxolid allows a thinner wall while maintaining fracture resistance equivalent to a 4.1 mm titanium implant. This is particularly valuable in narrow ridges and mandibular incisor sites without requiring bone augmentation.

Surface Technology

Neodent: Acqua Surface
  • Sand-blasted and acid-etched (SLA-type) base
  • Stored in isotonic saline to maintain hydrophilicity
  • Contact angle < 5° (superhydrophilic)
  • NeoPoros nano-topography increases protein adsorption
  • Accelerates osseointegration vs. conventional SLA by ~2 weeks
  • Available on all GM-family implants
Straumann: SLActive Surface
  • Sand-blasted and acid-etched (SLA) base
  • Chemically activated under nitrogen to preserve hydrophilicity
  • Contact angle < 5° (superhydrophilic)
  • Most extensively published accelerated surface (500+ studies)
  • 4-week osseointegration protocol validated in multicenter RCTs
  • Available on BLT standard and all Roxolid variants

Both surfaces are chemically comparable in their hydrophilicity mechanism and clinical outcome data. The SLActive has a longer publication record and more Level I RCT evidence. The Acqua surface is newer but well-supported by growing clinical data and shows equivalent accelerated healing in independent comparative studies. Neither surface has a clear clinical superiority in current literature — the primary differentiator is evidence volume.

Biomechanics and Crestal Bone Preservation

Crestal bone maintenance is determined by three primary factors: connection design (micro-gap), platform switching (stress distribution), and coronal implant geometry. Both systems address all three, but differently.

The Grand Morse 16° self-locking taper effectively eliminates the micro-gap that drives bacterial microleakage and bone-level inflammatory resorption observed with external hex and flat internal connections. Published 5-year data on the GM system show mean crestal bone loss of 0.2–0.4 mm, consistent with landmark studies on conical connections.

The Straumann BLT CrossFit connection yields similarly low crestal bone loss in multicenter studies (0.1–0.3 mm at 3 years in the pivotal BLT trials). The BLT coronal neck is smooth rather than microthreaded, which some clinicians prefer for simplified tissue management; others favor the Neodent microthread approach for its potential to stimulate coronal bone apposition.

Prosthetic Compatibility

Prosthetic Component Neodent GM Straumann BLT
Multi-unit abutments (MUA) Neodent GM MUA (0°, 17°, 30°) Straumann VarioBase / CARES MUA
Straight abutments Anatomic GM; Hybrid GM; Morse GM Variobase; Straumann CARES direct
Screw-retained Ti-bases Neodent Ti-base GM (BL & subgingival) Straumann Variobase (gold-colored screw)
Digital workflow compatibility Compatible with major CAD/CAM systems; scan bodies available Full CARES digital workflow; broadest third-party library
Full-arch MUA protocols All-on-X protocol validated with GM MUAs Extensive published All-on-X data with Straumann MUAs
Third-party abutment options Growing; major manufacturers support GM Widest library — virtually all abutment manufacturers

For complex restorative cases — especially in high-esthetic zones requiring customized emergence profiles or complex angulation corrections — Straumann's prosthetic ecosystem is broader and more deeply integrated with major lab workflows. For full-arch and standard single/multi-unit cases, the Neodent GM prosthetic library is fully adequate and continues to grow.

Evidence Base

Neodent Grand Morse

The GM connection and GM-family implants are supported by prospective studies, multicenter trials, and systematic reviews, primarily from Latin American and European centers. A 2020 multicenter study (Melo et al.) followed 1,200+ GM implants over 5 years, reporting a cumulative survival rate of 98.2%. A 2023 meta-analysis of conical connection implants including GM data confirmed significantly lower crestal bone loss versus external hex designs (weighted mean difference: −0.54 mm). The evidence base is strong but shorter in duration and smaller in total study count compared to Straumann.

Straumann BLT

Straumann carries the largest body of implant-specific clinical evidence of any manufacturer. The BLT system specifically has been studied in pivotal multicenter RCTs across North America, Europe, and Asia. The flagship BLT trial (Buser et al. 2017, updated 2022) followed BLT implants in 5 centers over 5 years, reporting 98.8% survival with <0.5 mm mean crestal bone loss. Multiple Level I studies validate SLActive accelerated healing and Roxolid in narrow sites. For clinicians who weight published evidence heavily in system selection, Straumann's record is difficult to match.

Price Comparison

This is often the deciding factor for high-volume practices. The Neodent GM system is significantly more affordable than Straumann BLT — typically 40–60% lower cost per implant unit at comparable US distributor pricing. This cost advantage scales substantially in full-arch cases requiring 4–6 implants per arch.

Cost Factor Neodent GM Straumann BLT
Relative implant price Affordable — best-in-class value Premium — reflects brand & evidence investment
Typical full-arch savings vs. Straumann $500–$1,200+ per arch Baseline
Abutment / component costs Lower — competitive third-party options growing Higher — but wide availability of all components
Per-implant value proposition Excellent for volume practices Justified for complex cases and study reliance
